Including voice as instrument, each in their field:

Cecil Taylor, Angelo Debarre, Domenico Scarlatti, Tatsuya Yoshida, Elvin Jones, Evan Parker, Karou Abe, Kenji Haino, Charlie Parker, John McLaughlin, Richie Blackmore, Jimi Hendrix, Les Claypool, Janick Top, Christian Vander, Mike Patton, Jean-Luc Ponty, Grigoraş Dinicu, Sergei Rachmaninov, Buckethead, Klaus Blazquiz, Freddie Mercury, Tommy Iommi, Agata, Sun Ra, Carla Kihlstead, Marc Ribot, Fred Frith, Jimmy Page, Bach, Busoni, Brian Chippendale, Art Tatum, Don Cherry, Tim Smolens, Timb Harris, All Members of Gentle Giant, Zeena Parkins, Jelly Roll Morton, and more.

It's hard to really name all when you listen a lot of prog/zuehl type bands that require every member to be virtuoso, or every inhumanly skilled classical composer or jazz pianist.

Terence Hill, as recently confirmed during an interview to an Italian TV talk-show, was offered the role but rejected it because he considered it "too violent". Dustin Hoffman and John Travolta declined the role for the same reason. When Al Pacino was considered for the role of John Rambo, he turned it down when his request that Rambo be more of a madman was rejected.
Al Pacino = God
