Not necessarily the most insane but Fantômas is some of the best avant-garde metal music ever.
And White Suns is uber intense Anything with Yamatsuka Eye in it fits the bill. Naked City is my favourite.

Grindcore is fun and I enjoy a lot of it (stuff like Gadget, Nasum, some Discordance Axis, Cyness).
Don't get Yacopsae, everything Frownland has posted, half of Mondo's contributions here. No problem with people enjoying it. Just doesn't do it for me. If you hear memorable riffs awesome, if you enjoy being awash in naked noise and rage fantastic. I take issue with being called a **** because I don't fellate bands that are purposefully off-putting in sound or aesthetic. Metal currently on my phone: The Axis of Perdition, Buried at Sea, Carcariass, Dodecahedron, Eikenskaden, Emperor, Gorguts, Hacksaw to the Throat, Kaamos, Krallic, Martyr, Melechesh, Negura Bunget, Nocturnus, Orphaned Land, Revelation of Doom, Sword, Thralldom, Vale of Pnath, Woods of Ypres, and Yyrkoon. Death, black, doom. I think my "cred" is solid - one does not need to fawn over every sub-genre.

I'm just saying Today is the Day has a very expansive sound loaded with outrageously catchy and/or powerful material. Like Willpower is totally not some kinda pointless blast fest. Some melodies on there hit hard. Sounds like Swervedriver playing extreme metal.
