Yep, it's because someone approved it by mistake. Normally when a user wants their avatar changed, they make a post in there that contains the image (as Bob had done), but it doesn't show up because that section of the forum requires moderator approval. It's the same with the Member Journals and anything posted in Editor's Pick. We see the posts in our CP and just approve or delete from there. I've since deleted the post you're talking about.

I don't know. It doesn't show who approved posts, only who deleted them. It happens. Especially because there is a function in the mod CP to approve all pending posts at once.
