Or you have more leechers than seeders. With a site like Mediafire or Megaupload you don't have to worry about such variables, as long as the site is still hosting the upload you can download it.

I always have luck with the remote servers.

As far as a missing password, a lot of times you can google the filename, and find the original uploader.

Or you could just find another one. Usually there are multiple copies out there.
