When I was working at a psychologist office, I'd write notes to the psychologists but would run out of space and have to turn over (my handwriting is big lol) and always put PTO because I was worried they would miss the extra information. I don't think many people I know that do it. It's just something I picked up along the way and I can't help but put it down. I am an anxious person sometimes which makes me feel like I have to relay everything exactly how I need it too.
