Not sure how to be any more specific with that assessment. Just looks like the separation of the signature and the avatar makes it look like there's a hand coming out of the head, and going under the forum, and coming back out.

I'm not totally sure. It's an album cover for In Never Out by pg.lost. I'll see if I can find an artist name, but I imagine it's just the band's art.
