The point of the whole thing is to illustrate an impossible choice. It is not relevant to a company decided that to skimp on materials in order to spend more in advertising.

A cost-benefit analysis is a type of trolley problem and it’s helpful to think of it that way because it brings home the reality of there being a literal price put on human life.

The defining attribute of the trolley problem is that it's a binary choice. Real life is almost entirely not binary choices.
