Usually, locomotives have some sort of control called the "dead man", which basically means the train will stop if the engineer is not pressing x button or pedal or whatever. I really don't know much about non-Canadian railways (and I haven't read much about this particular accident), but if he had dozed off, that would probably have prevented it.
