The photo used on the front cover of the album was a picture of former Mayhem vocalist Per Yngve "Dead" Ohlin after he committed suicide. He was found in the house where the bandmembers lived, his body was then discovered by Mayhem's guitarist Euronymous, who took pictures of the event. After Euronymous had been murdered by Varg vikernes, pictures of Dead's corpse were found by Euronymous' father. Euronymous' father was disgusted by the pictures and threw them in a trash can, where apparently someone found them and spread them throughout Norway and Sweden. Eventually the picture was used for the cover of the bootleg album Dawn of the Black Hearts.
