Caligola. I doubt a movie as mainstream as Requiem for a Dream could be among the most grotesque movies ever made. Granted, I haven't seen it, so I won't assume. However, try Ichi the Killer, Cannibal Holocaust, the Holy Mountain (you'll never find it, unless you really want it, and from what I hear it's not even worth seeing, so I wouldn't bother), or Man Bites Dog (one of my favorite movies) among others.
